Field Marshal Incident Report

1) Fill out all required sections

2) Attach the following under attachments:

- scanned copy of incident report

- body camera footage

- witness statements

- photos or video evidence

3) Click Submit

*Note that reports must be submitted within 24 hours of an incident unless the investigation is ongoing, reports will be reviewed by the director of safety for supplemental discipline, and we be viewable under 'incident history'
